You may obtain a copy of your medical records by bringing a USB to our office. There will be no charge for the records.

If you are coming to an IN-CLINIC appointment for sore throat, respiratory or sinus infections, you MUST TAKE A COVID TEST BEFORE you come into the clinic and have the results available for your appointment.
