All patients must present a valid OHIP card to access medical services.

Non OHIP patients will be charged according to OMA rates for medical services rendered.

​

Out of province patients will be billed for services that they can reimburse with their individual provincial health plans.

​

Results will not be given over the phone.

Patients should book an appointment to follow up for any results and tests.

​

We will not discuss a patients medical condition with an unauthorized individual.

Adolescents and adults over 16 years old must give written permission for parents/other individuals to access their records.

Aggressive and rude behaviour will not be tolerated at the clinic or on the phone.

​

The clinic and individual doctors reserve the right to discontinue any patient that has acted inappropriately to the staff or doctors.

 

Any patient suspected of double doctoring will be discontinued from the clinic.
